When You Arrive
Upon entering the office, we have some brief paperwork for you to fill out. The paperwork provides us with general information about your medical history and any current problems you are experiencing.
- All New Patients: New Patient Paperwork
- Worker’s Compensation Patients: Worker’s Compensation Intake Form
Consultation
Once the paperwork has been completed, you will have a consultation with the doctor to discuss your health-related problems, concerns, as well as potential care options.
Examination & History
In order to diagnose your problem, the doctor will ask various questions related to your condition. In addition, the doctor will perform several specialized tests to localize the affected areas and evaluate the extent of the problem.
Additional Testing
Your specific condition may require x-rays or an MRI to rule out more serious conditions and assist us in developing the most effective care plan for you.
Home Instructions
Prior to leaving, you may be given instructions on certain activities and/or procedures to followed at home. This may include ice or heat application instructions, avoidance of certain activities or positions, and various rehabilitative exercises and/or stretches you can do yourself.
Schedule Next Appointment
A follow up visit will be scheduled for the next day and a written treatment plan will be reviewed with the doctor to ensure a directed plan of care.
